The following two forms are available in the athletic office (or below as pdf downloads), and both must be submitted to the athletic office before an athlete can try, practice, or compete.

Picking Up Your Athlete?
We encourage all athletes to use the school district transportation when it is provided. However, you can make other arrangements if you plan ahead; please complete the "Travel Request and Authorization" form (below as a pdf or available inthe athletic office.)
